SAFETY CULTURE ASSESSMENT

A process which designed to objectively collect managers and workers sentiments and perceptions around existing HSE implementation. The data collected from this process will be analyzed by an expert team towards providing final report and recommendations. This process consists of two steps explained below.

This process will include review on documents, interview of leadership and selected employee, site visit which include safety observation and conversation, report development, and it will be closed by leadership workshop on final report

Safety and process improvement through capability development

Various workshops organized face to face and through online means to help in developing skills and stimulate behavior which support Safety Culture and Process Safety Management.

Process Safety Management (PSM) is a control system used for facilities so that potential hazards are identified, understood and well managed.

This module brings in detail the importance of having PSM and explains the 14 elements of Occupational Safety and Health Administration (OSHA) for the Operation, Maintenance, Engineering and other relevant personnel who exposed to the handling of dangerous chemicals.

Process Hazard Analysis (PHA) is a comprehensive study to determine potential hazards and its
consequences in a facility towards the development of a control system and recommendations to manage that as per applied standard and regulation.

This study is conducted by a team consists of personnel with combined knowledge and capabilities (in mechanical, electrical, instrument, chemical, civil, etc.) which have ability and capacity to make decision.

This workshop explains what is PHA, reviewing case studies, why PHA is needed, and the right process of conduction PHA.

Incident is something that is meant to be avoided, but when it happens, it is compulsory to conduct incident investigation so that the root cause can be identified. It is also important to develop recommendations and
action plans to ensure such incident will not happen again in the future.

To be able to conduct a proper investigation, every personnel involved in an incident investigation must have sufficient knowledge and techniques in collecting data which include document and physical reviews as well as interviews to prove the hypotheses of the cause of an incident with hard evidence, instead of by the subjective opinion without evidence.

This workshop is designed to ensure participants understanding on the importance of incident investigation and why it is needed. It brings participants through the steps of investigations, interview techniques, and the process of developing relevant recommendations. There are also number of case studies reviewed during this workshop.

Implementation of mechanical integrity program helps facility to run properly. It keeps the integrity of equipment and ensure that all work properly as needed and designed.

Mechanical integrity is not only aiming to anticipate failure in the process of operation which could lead to a catastrophic incident, but also to keep the whole performance of mechanical assets and avoid unplanned downtime.

This workshop is designed to give participant an understanding of mechanical integrity. It reviews some of case studies showing catastrophic incident due to failure in implementation of mechanical integrity program in a proper formal and structured way.

Contractors are non-organic part of a facility’s operation. Depending on the nature of the contract, there could be risk of shortcut and improper cost cutting.

To ensure contractors work in accordance with the required HSE standard, it is fundamental to have a
system to manage contractors starting from the prequalification stage down to the review of result and performance.

This workshop explains the importance of a proper CSMS including the steps of developing one. There are also number of case studies reviewed during this workshop to help in better understanding.
